Services focused on functional capacity, daily living skills, motor coordination, or participation goals identified in your referral.
Support including mobility assessment, strength and balance programs, pain management, or rehabilitation goals.
Sessions structured around identified mental health concerns, delivered under a Mental Health Care Plan.
Sessions focused on emotional wellbeing, coping strategies, and goal-directed support within your approved care plan.
Nutrition support for concerns including cardiovascular conditions, weight management, and diabetes management under a Chronic Disease Management Plan.
